Incident Report Form
Drivers must immediately notify Dispatch by phone at the time of any accident, passenger injury, vehicle damage, safety concern, complaint, mechanical failure, or unusual event occurring during the course of duty. Verbal notification to Dispatch is required before completing this written report.
This Incident Report Form must then be completed as soon as safely possible following the event and before the end of the shift in which the incident occurred. Prompt reporting ensures passenger safety, regulatory compliance, insurance protection, and proper broker notification.
All incidents—regardless of perceived severity—must be documented. Drivers are responsible for providing a complete, factual, and accurate account of events, including actions taken and notifications made.
Failure to immediately notify Dispatch or failure to submit a timely and truthful report may result in disciplinary action up to and including suspension of driving privileges or termination.
