PRE-TRIP VEHICLE INSPECTION FORM

This inspection must be completed in full prior to operating any company vehicle or initiating the first transport of the shift. The purpose of this inspection is to ensure vehicle safety, regulatory compliance, and the protection of our passengers, drivers, and company assets.

Drivers are responsible for accurately reporting any defects, warning lights, equipment deficiencies, or safety concerns before placing the vehicle into service. Vehicles deemed unsafe must not be operated under any circumstances.

Failure to complete this form prior to transport, or submission of inaccurate information, may result in disciplinary action up to and including suspension of driving privileges.
